Job Description:  Interfaith Human Services (IHS) Case Manager

Position Summary:  The primary responsibility of the IHS Case Manager is to coordinate and stabilize a client’s basic needs and finances. Case management is primarily conducted through IHS’s Financial Care Programs which include: The Money Management Program and The Social Security Representative Payee Program, and the Financial Care for Veterans Program.

Money Management Program

  • Receive referrals from Centre County human service agencies, IHS board member faith communities, and self-referrals from any Centre County resident who seeks financial management assistance.
  • Conduct initial intake interviews to assess client needs and explain program guidelines and process, fees for services, and sponsorship program (if applicable)
  • Prepare applications and client contracts
  • Schedule client appointments and establish financial accounts, as applicable
  • Establish client household goals: review and update as needed but at least every 6 months
  • Track household progress toward financial stability and maintain accurate records
  • Make applicable referrals, collaborating with other county agencies, to stabilize household basic needs
  • Maintain timesheet and fees/scholarships for monthly billing
  • Coordinate internal financial assistance process with the office manager
  • Track visits and maintain and complete all documents, forms, reports, and case notes required by contracts, grants, and IHS policy
  • Work collaboratively as part of the IHS team, updating the executive director on client issues

Social Security Representative Payee Program

  • Receive referrals from Social Security and Centre County human service agencies
  • Assess client needs at initial intake interview and explain program guidelines and process
  • Prepare applications and client contracts
  • Submit new client applications to the executive director for review, approval, and signature
  • Establish financial accounts as applicable and inform the client of status
  • Establish client household goals: review and update as needed but at least every 6 months
  • Track visits and maintain all documents and case notes required by Social Security Administration, other contracts, grants, and IHS policy
  • Provide monthly financial statements to clients, including copies of any statements
  • Coordinate internal financial assistance process to include providing checkbook and current bills to the office manager
  • Make applicable referrals to other county agencies to stabilize household basic needs
  • Work collaboratively as part of the IHS team, updating the executive director on client issues.

Financial Care for Veterans Program

  • Receive referrals from the Veterans Affairs Office and other appropriate organizations and agencies
  • Assess client needs at initial intake interview and explain program guidelines and process
  • Prepare applications and client contracts to meet VA and IHS requirements
  • Schedule client appointments and establish financial accounts, as applicable
  • Submit new client applications to the executive director for review, approval, and signature
  • Establish financial accounts as applicable and inform client of status
  • Track visits and maintain all documents and case notes required by the Veterans Affairs office, other contracts, grants, and IHS policy
  • Provide monthly financial statements to clients, including copies of any statements
  • Coordinate internal financial assistance process to include providing checkbook and current bills to the office manager
  • Make applicable referrals to stabilize household basic needs through the Centre County Office of Veterans Affairs
  • Work collaboratively as part of the IHS team, updating the executive director on client issues.

Additional Requirements:

  • Bachelors degree in sociology, human services or applicable field
  • Strong background in finances
  • Ability to be bonded for financial liability
  • Ability to work with complex family situations
  • Ability to travel (must provide transportation) to client homes (as applicable)
  • Valid PA driver’s license, current registration and insurance
  • Position is 40 hours per week, with core hours of 9-4.
